If you’ve noticed your filler doesn’t look right — and it’s been bothering you more and more — there’s a good chance what you’re seeing is filler migration. It’s one of the most common reasons patients come to us for dissolving, and understanding what it is can help you feel a lot less panicked about it.

Filler migration happens when hyaluronic acid filler moves away from where it was originally injected. It doesn’t mean your injector did something wrong necessarily — though technique is a factor — and it doesn’t mean something is medically dangerous. It means the filler has shifted to a location where it shouldn’t be, creating visible changes you didn’t sign up for.

What migration looks like: Lips that look puffy or swollen above the lip border (sometimes called a “filler shelf” or “filler mustache”). Under-eye fullness that appeared months after cheek filler. A heaviness or unnatural contour that wasn’t there right after your appointment. Lumps or ridges you can feel under the skin.

Why it happens: Too much filler placed in one area at once. Products that are too soft for the treatment zone. Natural facial movement pushing filler along tissue planes over time. And sometimes — honestly — it just happens, even when everything was done correctly. Filler is placed in a living, moving face, and tissue doesn’t always behave the way anyone plans.

The good news: Migration doesn’t mean the filler is broken or that you need surgery. It means the filler needs to be dissolved and — if you choose — replaced with the right product, in the right amount, by the right hands. That’s exactly what we do.

How Dissolving Filler Works

The treatment is straightforward. We use an enzyme called hyaluronidase — a naturally occurring substance your body already produces — to break down hyaluronic acid filler.

When hyaluronidase is injected into the area where unwanted filler sits, it breaks the chemical bonds that hold the filler gel together. The filler loses its structure, softens, and your body absorbs the remnants through its normal metabolic processes. What was a visible lump, ridge, or overcorrection starts dissolving within hours and is typically resolved within 24 to 72 hours.

It’s not a new or experimental treatment. Hyaluronidase has been used in medicine for decades — long before aesthetic fillers existed. In the context of filler correction, it’s one of the safest and most reliable tools we have.

One important caveat: hyaluronidase only dissolves hyaluronic acid fillers. That includes the Juvéderm and Restylane families — the most commonly used fillers in the US. It does not dissolve Sculptra (which is made from poly-L-lactic acid), Radiesse (calcium hydroxylapatite), or silicone. After dissolving, many patients choose to have their lips re-injected once the area has settled — this time with a more conservative approach that gives them the result they actually wanted. Others decide they prefer their lips without filler. Both choices are equally valid.

Dissolving Under-Eye Filler

Under-eye filler correction is one of the most delicate dissolving procedures we perform. The tissue around the eyes is thin, vascular, and unforgiving — which is why this area is both one of the most requested and one of the most technically demanding for filler placement.

If you’re seeing puffiness, Tyndall effect (a bluish tint under the skin), or visible lumps beneath your eyes, hyaluronidase can address it. This area often requires a more conservative, staged approach — sometimes one session is enough, sometimes we dissolve gradually over two visits to ensure the right amount is removed without over-correcting.

Cheek filler that’s migrated or been overcorrected can create a heavy, puffy look that doesn’t match the rest of your face — or a visible shelf where the filler sits unnaturally beneath the skin. What to Expect During Your Appointment

The consultation. This is the most important part. Your provider will assess the area, discuss what you’re seeing and feeling, determine what type of filler is present (not all filler can be dissolved), and create a plan. If you got your filler elsewhere and don’t have records, that’s completely fine — we can work with what we see.

We’ll also be honest with you about expectations. Sometimes what looks like a filler problem is actually swelling, weight change, or natural aging — and dissolving won’t address those. We’d rather tell you that upfront than treat something that won’t give you the result you’re looking for.

The treatment. Hyaluronidase is injected directly into the area where the unwanted filler sits. The appointment takes about 15 to 30 minutes depending on how many areas are being treated. You’ll feel a slight pinch — similar to the filler injection itself.

Immediately after. Expect temporary swelling. This is normal and expected — it can actually make the area look worse before it looks better. Don’t panic. The swelling resolves within 24 to 48 hours, and the dissolving effect becomes visible as the swelling subsides.

Within 24 to 72 hours. The filler breaks down and your body absorbs it. Most patients see significant improvement within a day or two. The full effect is usually visible at the one- to two-week mark.

If more is needed. Sometimes one session is enough — especially for recent filler or smaller amounts. But for deeper, older, or larger-volume filler, a second (or occasionally third) session may be needed, spaced about two weeks apart. Your provider will assess after the first treatment and let you know.

After Dissolving — What Comes Next

Once the unwanted filler is dissolved, you have options.

If you want to re-inject: We recommend waiting at least two weeks before placing new filler. This allows the area to fully settle, any residual swelling to resolve, and your natural tissue contour to reveal itself. Starting fresh on a clean canvas — rather than building on top of old filler — almost always produces a better result.

If you’re done with filler: That’s a perfectly valid choice. Some patients dissolve and discover they like their natural contours more than they expected. No one at Illume is going to push you toward more filler if that’s not what you want.
